The new tensioner will have a pin holding it retracted. You will pull the pin when you get it in place and let the tensioner extend. The battery hold down bolt is used to compress the old tensioner so you can relieve the tension on the belt so you can remove the tensioner. I have just used a bolt instead of the battery hold down.

If it is rattling you may be able to just remove it without compressing it. Not sure.

Since tb is only 5000km old do I need to replace as it was slapping around due to failed tensioner?
The timing belt is the one single maintenance item that can destroy your engine in an instance if not cared for properly. We need our timing belts to last 90k+ miles between service. A timing belt slapping around (being stretched and jerked) is an abused belt. It's a no brainer for me. Replace the abused belt. I've rolled the dice many times on Honda reliability, but I couldn't sleep at night if I just replaced a failed tensioner.
I'm glad you brought this question to the table.
