2003 Pilot, ~137000mi
D started blinking yesterday. Started again this am after driving about 5 minutes. The manual says it has to do with transmission sensors, switches, etc and don't drive aggressively. I had the code pulled @ auto part store and got P1491 - a proprietary code - which turns out to be "EGR Valve Lift Insufficient Detected". I know the EGR valve has some link to the transmission because I replaced it a few years ago to get rid of a "shift shudder" (but it lit up the Check Engine Light that time). So, should I replace the EGR valve again or is it not related to the blinking D?

You have the ports that tend to clog up. I would clean them first and see what happens. May need a new valve too, but if the ports are plugged up it won't matter.
