You use the tire iron. If you bought your Pilot new, go back to the dealer and get it. If you bought it used, you can use any 3/4" tire iron. I use this with a 1/2" impact socket. It's from Sears.

What model year is your Pilot?
like krygny said, you have to use the lug nut wrench supplied with the Pilot.

In my '05, the bolt for the spare tire hoist doesn't have a standard 19mm hex head. It looks more like some type of specialty allen wrench/torx shape. The lug nut wrench that came with my Pilot has an extra indentation that fits the top of the bolt. Any generic tire iron or socket wrench won't work. I assume that the '05 and newer models are like this. I'm not sure about the '04 models, but according to photos I've seen on this site the '03s use a 19mm hex head bolt.

If your Pilot didn't come with the standard lug nut wrench, make sure you order one or pick one up from a Honda dealer.

You can get the 19mm lug wrench with jack rods kit for about $15 from your Honda dealer. Comes complete with a genuine vinyl case.

...
Yes, 19 mm or 3/4", not 1/2". The socket is 1/2" drive .
